Property Damage Claim Form

• If the claim is allowed, a general release is sent to the customer for execution. Once the signed release is received back, the Commission will process your claim for payment.

The Commission requests the following information, if available:

• Name, address, phone number and email address;
• Date, time and location of incident;
• Make, model and year of vehicle;
• A description of what happened, and the damages incurred;
• Cash or E-ZPass customer;
• The OSHP Traffic Crash Report number if applicable;
• A copy of the claimant’s certificate of insurance with deductible;
• The repair invoice, estimate, or quote.
