The Commission requests the following information, if available:
- Name, address, phone number and email address;
- Date, time and location of incident;
- Make, model and year of vehicle;
- A description of what happened, and the damages incurred;
- Cash or E-ZPass customer;
- The OSHP Traffic Crash Report number if applicable;
- A copy of the claimant’s certificate of insurance with deductible;
- The repair invoice, estimate, or quote.
